تجزیه و تحلیل علت ریشه ای برای سرریز مخزن به دلیل مشکل دریچه
این حادثه در مخزنی رخ داده بود که در آن آب پر شده بود. اگر مخزن دارای مایع خطرناک دیگری بود، ممکن بود منجر به مرگ شود.
بیان مسأله
در یکی از کارخانه های یک پالایشگاه مخزنی برای ذخیره آب در آن تعبیه شد. ورودی تغذیه مخزن با استفاده از شیر خاموش/روشن (On-Off) (بدنه پروانه و قفسه و محرک پینیون) کنترل شد. عملکرد خرابی دریچه On-Off مشکل در بسته شدن بود.
یک روز، اپراتور حاضر در محوطه به ما اطلاع داد که شیر از قسمت محرک یا اکچوایتور دارای مقداری نشتی است. تیم ابزار دقیق شیر را بررسی کردند و متوجه شدند که تعمیرات اساسی محرک مورد نیاز بود.
روز بعد، محرک به منظور تعمیرات اساسی رها شد. اما لازمه عملیات این بود که شیر در حالت باز باشد زیرا مخزن سطح پایینی داشت و پر کردن مخزن در حال انجام بود.
بنابراین، در شرایط باز سوپاپ، محرک با دادن هوای خارجی به محرک رها شد تا محرک باز بماند. پس از تعمیرات اساسی محرک ، محرک مجدداً نصب شد. محرک در شیفت دیگری توسط شخص دیگری دوباره نصب شد.
پس از چند ساعت در حین دور میدانی، اپراتور میدانی مشاهده کرد که مخزن پر شده است. بلافاصله ورودی مخزن جدا شد.
هنگامی که محرک پس از کار تعمیر و نگهداری مجدداً نصب شد، سطح مخزن 82٪ بود. بالای 80% شیر ورودی تغذیه مخزن در وضعیت بسته است. هنگامی که سطح به زیر 25٪ می رسد، شیر ورودی تغذیه دوباره باز می شود.

بنابراین، دلیل احتمالی سرریز شدن مخزن چه می تواند باشد؟
بررسی ها نشان داد که فرستنده سطح درست کار می کند. وقتی مخزن شروع به سرریز شدن کرده، فرستنده سطح 101% را نشان می داده که همین مقدار تایید شد.
مشکل احتمالی 2: آیا دریچه On-Off هنگامی که سطح بالای 80 درصد است بسته نمی شود؟
بررسی بازخوردها و گزارشات ثبت شده از دریچه On-Off بسته بودن دریچه را نشان میداد.
مشکل احتمالی 3: آیا سوئیچ مجاورت خراب است و هنگامی که شیر باز بوده بسته بودن شیر را نشان میداده است؟
عملکرد سوئیچ مجاورت نیز بررسی شد و سالم بودن سوئیچ مجاورت را گزارش داد.
مشکل احتمالی 4: آیا شیر On-Off اصلی دارای نشتی است و عملکردش ناقص می باشد؟
احتمال وجود این مشکل وجود دارد.
مشکل احتمالی 5: آیا در شیر بای نشتی وجود دارد؟
هنگامی که دریچه ایزوله برای دریچه On-Off بسته شد، سرریز مخزن متوقف شد. بنابراین هیچ مشکلی در مورد شیر خط بای پس وجود ندارد.
این نیز می تواند یکی از دلایل باشد.
مشاهدات پس از ارسال شیر یا دریچه On-Off برای تعمیر و نگهداری:
- محرک برای بررسی های بیشتر کنار گذاشته شد. هنگام بازکردن محرک، دریچه در حالت بسته قرارداشت.
- در حالی که در حین بازکردن بدنه برای بررسی مشکل عبور، بدنه اصلی شیر در حالت باز مشاهده شد.
این بدان معناست که جهت نصب محرک نادرست بوده است.
چه چیزی اشتباه پیش رفته و چرا این اتفاق افتاده است؟
در حین کار، زمانی که محرک از روی خط باز شده بود شیر در مقعیت باز رها شده بود. هنگام نصب مجدد محرک، تکنسین محرک را در شرایط بسته نصب کرده، در حالی که بدنه شیر اصلی همچنان در حالت باز بوده است.
از آنجایی که تکنسین از این موضوع بی خبر بوده و همچنین بدنه یک جفت به شکل مربع داشته است تکنسین متوجه اشتباه نشده و با برداشتن کوپلر وضعیت بدنه را بررسی نکرده بوده است.
تکنسین محرک را در حالت بسته نصب کرد. سپس بازخورد را تنظیم کرد و شیر را برای عملیات تحویل داد.
با توجه به این حادثه باید دانست که جهت هر ابزار قبل از برداشتن باید علامت گذاری شود و همچنین باید نحوه تحویل مناسب کارها مشخص گردد.
